Services

At Community Personal Care, we involve you, your family, caregiver or designee if desired, in the formulation of an individualized Plan of Care (POC). All services are provided under the supervision of a licensed Registered Nurse who will work with you to develop a POC based on your needs, environment, and personal goals and wishes. The POC is reviewed and updated as needed, based on your changing needs and goals. The Aide or Companion is responsible for following the POC to assist with your daily needs and help you meet your personal goals.

We can provide an array of services in your home based on your needs. Our services include Personal Care, Companion Care, Respite Care and Private Duty.

Personal Care (Medicaid Program)
This service provides eligible individuals with a Personal Care Aide to assist with daily hygiene care, meals and light housekeeping.

Companion Care
This service provides eligible individuals with a Companion to help with housekeeping, laundry, and meals. Personal hygiene assistance is also available.

Respite Care (Medicaid Program)
This service provides an Aide for the temporary, periodic or routine relief for the primary caregiver.

Private Duty
This service provides individuals with a Personal Care Aide/Certified Nurses Assistant to assist with daily hygiene care, meals and light housekeeping and in certain cases may also check vital signs and assist with socialization.


Personal Care

Our Private Duty/Personal Care services are designed to enable you to remain in your home or stay with a family member while maintaining as much independence as possible. These supportive services can be arranged to fit your schedule including round the clock care.  

Upon admission, a Registered Nurse will meet with you and your family or caregiver to assess your needs and, when necessary, consult with your physician(s), social workers, hospitals, local health department or nursing home staff. Next, we develop an individualized Plan of Care (POC) to manage your daily needs. We match your needs with a qualified Personal Care Aide (PCA) and provide you with an opportunity to meet the PCA and participate in the selection process. The POC and any special requirements are reviewed with the PCA prior to the start of care.

Once care begins, we continue our unique system of personalized contact through telephone check-in and regular home visits performed by the RN assigned to manage the case. In addition, we maintain an open and continual line of communication with family members or caregivers because your satisfaction is our first priority.

Our Personal Care Aides are professionally trained by a Registered Nurse to provide supportive services that are essential to maintaining optimal level of care. These services include but are not limited to:

  • Bathe or assist with bathing
  • Dress or assist with dressing
  • Groom or assist with grooming
  • Assist with self-administered medications
  • Assist with ambulation
  • Assist with transferring from bed to chair, etc.
  • Assist with toileting
  • Feed or assist with feeding
  • Position in bed
  • Change linen
  • Clean up immediate surroundings
  • Remove/dispose of trash
  • Laundry
  • Prepare meals
  • Make and accompany you to appointments

The services you receive will be based on your individualized Plan of Care.

Companion Care

Sometimes you just need little support with the basics and that is what our Companion Care services are designed to do. Our Companions offer a level of support and interaction that helps you remain engaged in life. Our successful Companion program reduces the feelings of isolation and encourages activities that stimulate thinking and promote physical well-being. Moreover, the socialization that Companions provide will keep you connected emotionally and improve your overall well being.

Our Companion services include:

  • Washing dishes, load/unload dishwasher
  • Vacuum and sweeping floors
  • Surface dusting
  • Remove/dispose of trash
  • Wiping down appliances
  • Wash/dry clothing and linen
  • Prepare light meals
  • Limited errands/shopping

Respite (Medicaid Program)

The responsibility of caring for an elderly or disabled loved one can be extremely stressful. It is a challenge to find time for yourself while balancing your needs and those of your loved one. Respite Care is designed for the family member or primary caregiver and provides relief support so that you, the caregiver, can take a break. Being a better you enables you to be a better caregiver to the ones you love.
